The fingerboard fell off and there seems to be a small gap in the heel....

The gap is there by design , it is a dovetail joint . This looks like it has been overly stressed by Humidity or soaked in water.

There does appear to many issues here. Lots of moisture for sure and is the action really that high?

I'm with Alex, an emergency canoe paddle or perhaps a planter box with a story

I've seen this a few times before. The most likely cause is that the luthier used 150 gram strength hide glue instead of 192. The heel crack should buff right out.

My thoughts exactly, and the guitar spent five years laying flat on the floor of said building.

Looks like the fretboard pulled that rosette decal right up with it!
