What do you think of this? Is it suitable to drill tuner holes for a slot head?

it certainly is. make it out of aluminum, delrin, or acrylic and use drill bushings and you're ready to rock. you might want to put an adjustable stop on one side of it to register against the end of your headstock for repeatability. i personally don't use one but they are common.

Make it out of a hard hardwood and it should last for . . . who knows. I once made one out of Beech and it did some 20 odd Guitars. It's still going but I broke down and bought the LMI one. Candle wax the side of the drill bit and it will help to preserve the holes. You want at least 2 " material on the drill guide, if you can.

Goes without saying but I'll say it anyway - use a drill press if you have one to drill the three tuner holes.

Make it out of wood, but if you sink a steel insert it should still last. But you'll want a drill press anyways (a doweling jig is still good to have even if you have a drill press, if not for repeatability)

I do have a drill press and was considering just lining up the headstock and drilling with it. I have not made a slot head yet. I'm thinking my next build to be a 000 12 fret with a slot head and thought that perhap it might be better to use a dowling jig rather than simply the drill press alone... On the other hand, I can see that the head plate (and possibly backstrap) might be situated such that the center may not be exactly where I want to put the hole... For that reason, it might be better not to use a dowling jig that centers the hole.

You can still use a dowel type jig like the one you gave the link to, to drill off center holes, Steve. Just put a spacer on one side of the jig. I think if you make the spacer twice as thick as the desired off set, you'll hit your spot.
